New patients, in order to finalize your registration process, please arrive 15 minutes before your scheduled appointment time. Your New Patient paperwork MUST be completed prior to your arrival (click here to download). If you cannot complete your New Patient paperwork prior to arrival, please arrive 30 minutesbefore your scheduled appointment time. Otherwise, your appointment may be rescheduled.
